If you would like to enroll in the Portage County Safety Council for the first time, please complete and return the Enrollment Form and the Annual Membership Dues Invoice. If you’re currently an active member, simply return a completed FY20 invoice.
- Rebate eligible companies must enroll by July 31, 2019, in order to qualify for the FY20 rebate opportunity.
- Companies who were active in the previous program/fiscal year DO NOT need to complete an enrollment form.
- Enrolling companies must have their Annual Membership Dues paid prior to becoming rebate eligible member. Click here for more info.
- By completing an Enrollment Form you are agreeing to meet all the requirements of the Ohio Safety Council Program including but not limited to accurately completing Semi-Annual Reports.
